ONLY faculty and staff of Appalachian State University are eligible to use this form for placing materials on Reserves.

  • We have simplified this request form -- you may use this same form to request electronic or physical items.
  • You can make up to 5 requests for one course using this form. If you have more than 5, please submit another form.
  • Please complete all sections.
  • NOTE: Incomplete/incorrect information causes delays in processing.
  • Notification of completion of processing of material is via e-mail.
